Well organised, fluent responses can be mistaken for expertise, increasing the risk of misplaced confidence in recommendations that reflect design patterns rather than clinical evaluation. For mental health, healthcare practice, and public policy, this means AI outputs must be treated as psychological interventions as well as information tools, with greater attention to how presentation steers judgement and behaviour.




Artificial intelligence is rapidly becoming a first stop for healthcare decisions. Instead of navigating dozens of websites or calling multiple clinics, many people now ask AI systems direct questions about where to seek treatment, which specialist to consult, or which clinic operates locally. This shift has happened quietly, but its implications are substantial.

Most debate around AI in healthcare centres on accuracy. That focus is justified, but incomplete. Accuracy alone does not determine whether advice is trusted or followed. In healthcare, presentation plays a decisive role. How information is structured, framed, and delivered strongly influences how it is interpreted, particularly when people feel uncertain or anxious.

Psychological research has long shown that human decision making is not purely rational. People rely on mental shortcuts, especially under pressure. In health related contexts, clarity and order often stand in for expertise. When AI systems present recommendations, they do not merely convey information. They shape perception.

Structure shapes perceived authority

The organisation of information has a powerful effect on credibility. Clear segmentation, logical sequencing, and concise summaries reduce cognitive effort and create a sense of control. Poorly structured text does the opposite. It increases friction and uncertainty, even when the underlying facts are identical.

In healthcare settings, this effect is amplified. Patients are often dealing with unfamiliar terminology, emotional strain, or time pressure. A recommendation that appears orderly and composed is more likely to be trusted than one that feels vague or cluttered. This is not because it is better informed, but because it is easier to process.

Psychologists refer to this as cognitive fluency. Information that flows smoothly is often judged as more accurate and more authoritative. When AI systems generate recommendations using lists, neutral language, and confident framing, they tap directly into this bias.

What recent observations reveal

A descriptive analysis examined how AI systems present healthcare recommendations when users asked for clinics in Spain. More than 1,500 AI generated responses were reviewed across multiple cities and healthcare categories. The focus was not on whether the recommendations were correct, but on how they were constructed.

The findings were revealing. The phrasing of the user’s question influenced the structure of the response more than the clinic, location, or medical category. Open ended questions tended to produce longer answers with contextual explanation and multiple options. Requests framed as quick or direct led to shorter, more decisive sounding recommendations with minimal detail.

Different AI systems also showed stable stylistic patterns regardless of context. This suggests that many recommendations are shaped less by an assessment of healthcare quality and more by underlying design choices and linguistic templates.

From a psychological standpoint, this matters. If people interpret structure as authority, they may be responding primarily to formatting and tone rather than to substantive evaluation.

Cognitive load, fluency, and misplaced trust

Cognitive load theory helps explain why these patterns are influential. When information is easy to process, people experience a sense of fluency. That fluency is often mistaken for reliability. In high stakes domains such as healthcare, the risk of this misinterpretation increases.

AI systems are particularly effective at producing fluent text. Ordered lists, balanced language, and polished summaries convey confidence, even when the system is not weighing clinical outcomes or patient safety data. As a result, users may place a level of trust in the recommendation that exceeds what the underlying process warrants.

This does not imply that AI advice is deceptive. It highlights that its psychological impact extends beyond content accuracy. Presentation itself becomes a persuasive force.

The role of the user’s question

Another overlooked factor is the user’s own role in shaping the response. The same AI system can sound cautious, authoritative, or decisive depending entirely on how the question is phrased. Asking for “the best clinic” invites a different structure than asking for “options” or “a comparison”.

From a behavioural perspective, authority is co constructed through interaction. Users unintentionally influence the tone, confidence, and perceived weight of the answer they receive. Without recognising this dynamic, people may attribute judgement or endorsement to outputs that are largely driven by linguistic cues.

Why this matters for healthcare decisions

Healthcare decisions are never purely informational. They are shaped by trust, anxiety, and perceived expertise. When AI systems mediate these decisions, their structural choices become part of the decision environment itself.

This does not require rejecting AI as a tool. It requires understanding how it influences perception. Before asking whether AI should recommend healthcare providers, a more basic question needs attention. How do people interpret and respond to the way AI presents information?

Recognising that mechanism is essential for using these systems critically and responsibly, especially in contexts where trust can have real consequences.
